Gujarat State Network Of People Living With HIV/AIDS (GSNP+) Inviting application for the post of District Sahayogi, for GFATM, Project “SAMAGRA”

GSNP+ is a community-based Organization formed in 2003, by people living with HIV (PLHIV) to improve the quality of life of infected and affected people with HIV. GSNP+ works in entire Gujarat, Madhya Pradesh and Maharashtra through CBO and NGO partners to achieve organizations vision and mission. GSNP+ is affiliated with National Coalition of People Living With HIV in India (NCPI+) a national level federation of PLHIV CBO’s.

To improve the quality of life and provide a sense of belonging to people living with HIV and their families for full active participation in society, and reduce further HIV transmission.

SAMAGRA
GSNP+ has been selected as a SR for the Global Fund project named ‘SAMAGRA’. This program is being implemented with a key objective to improve the survival and quality of life of PLHIV by establishing 41 Care & Support Centers (CSCs) in Maharashtra state and providing technical support for Mumbai and Goa SACS supported CSCs to improve treatment outcomes and meet the needs of PLHIV, including those who are members of high-risk groups, women and children.

Key Responsibilities

1. Strategic Coordination and Program Implementation

  • Support district-level implementation of MAS activities.
  • Coordinate with DACO, CTHO DAPCU, ART Centres, ICTCs, Medical Colleges, District Hospitals, CSC’s and Community-Based Organizations (CBOs).
  • Coordination with District collector, District Development officer, Commissioner of Municipal commissioner, Deputy health Commissioner, etc
  • Ensure alignment of district activities with MSACS and NACO guidelines.

2. First 95: HIV Case Identification

  • Monitor HIV testing coverage among priority populations and high-burden areas.
  • Support identification of testing gaps and low-performing facilities.
  • Coordinate outreach and community-led HIV screening initiatives.
  • Facilitate index testing, partner notification, and risk-based screening strategies.
  • Track district progress on HIV positivity yield and case detection.

3. Second 95: Treatment Linkage and Retention

  • Track and monitor treatment initiation within recommended timelines.
  • Support follow-up of missed appointments, treatment interruptions, and Lost-to-Follow-Up (LFU) clients. Coordinate with outreach workers, peer educators, and Positive Networks to improve retention in care.
  • Facilitate district reviews on treatment continuity and adherence support.

4. Third 99: Viral Load Suppression

  • Track and monitor viral load testing coverage and timely sample collection and VL suppression rates at district and facility levels.
  • Identify unsuppressed VL clients and facilitate Enhanced Adherence Counselling (EAC) in close coordination with CSC’s. Coordinate follow-up and repeat VL testing as per national guidelines.
  • Support district strategies to improve treatment outcomes and viral suppression.

5. Supportive Supervision

  • Conduct supportive supervision visits to health facilities.
  • Facilitate dissemination of updated guidelines and best practices.
  • Document innovations and success stories from the district.

6. Review Meetings and Stakeholder Engagement

  • Organise district-level review meetings under Mobilisation for AIDS Suraksha.
  • Follow up on decisions and action points from district and state review meetings.
  • Follow up on NACO direction for state & district specific strategy.
  • Coordinate with government departments, development partners, and civil society stakeholders.
  • Participate in a meeting organised by SACS/DACS, such as DAPCC, ART CSC coordination meeting, TI ART coordination meeting, etc.
  • Coordinate with DAPCC Member

Key Deliverables

  • Improved district performance on 95:95:99 indicators.
  • Improved viral load testing coverage and viral suppression rates.
  • Functional district-level coordination and review mechanisms.
  • Documentation of best practices and success stories.

Qualifications

  • Master's degree in Social Work, Public Health, Sociology, Community Development, Life Sciences, or related discipline.
  • Minimum 2–3 years of experience in public health, HIV/STI, TB, community health, or development sector programs, Community network
  • Computer literate
  • Proficiency in MS Office, data analysis, and report writing.
  • Fluency in Marathi, Hindi, and English.
  • A preference should be given to a Person Living with HIV. (PLHIV)/ Key Population Living with HIV (KPPLHIV).

Core Competencies

  • Strong communication and interpersonal skills, with coordination and stakeholder management.
  • Ability to work independently and travel extensively within the district.

Travel Requirement

Extensive travel within the assigned district for facility visits, community outreach, review meetings, and supportive supervision.

Performance Indicators

  • Achievement of district 95:95:99 milestones.
  • ART linkage and retention rates.
  • Viral load testing and suppression coverage.
  • Timeliness and quality of reporting.
  • Number of supportive supervision visits conducted.
  • Closure of district-level action points and service delivery gaps.
